Community Health Needs Assessment (CHNA)

Thank you for taking the CHNA survey!  

The CHNA survey reflects our region’s enhanced collaborative approach among community partners to better understand and address top health issues facing our residents. Through this collaboration we’ve convened a pathway to implement sustainable health improvement programs in each community. More than 10,000 residents took this year’s survey, sharing feedback on topics such as behavioral and medical health, education, social determinants of health and much more. The full 2021 report and key findings will be published later this year. 

The CHNA is completed every three years to better understand the overall health of our community.
